The VA Finger Lakes Healthcare System, NY collaborates with the Finger Lakes Geriatric Education Center at the University of Rochester under funding by the Health Resources and Services Administration (HRSA) Bureau of Health Workforce through the Geriatric Workforce Enhancement Program (Grant # U1QHP28738).  This collaboration supports interprofessional, team-based, patient-centered health care for older adults through integration into primary care delivery systems and through outreach and education for patients, families, and caregivers to improve the health of older veterans.
